Prescription medications: "All medications prescribed to you cannot be returned to us" and "are non-refundable, regardless of usage, program duration, or cancellation of membership" (Returns & Refunds policy) — this covers medication already shipped. Terms & Conditions Section 16 provides the one carve-out: a partial refund (minus a $50-100/portion administrative fee) specifically for the UNSHIPPED remainder of medication when a multi-month program is cancelled early (see cancellation_notes), waived for documented medical ineligibility. Scales/medical devices: non-refundable except approved damage/loss claims (72-hour reporting window), which receive a replacement credit, not cash. To cancel a scheduled visit, patients may request modification/cancellation any time before the visit to avoid visit fees; visit fees (where applicable, to select unspecified services) are non-refundable once a consultation is completed. To cancel Mental Health, Insomnia, or Hypertension memberships, or automatic recurring shipments/refills (e.g. ED, Birth Control), patients must request cancellation at least 48 hours before the next renewal/refill/billing date; cancellation takes effect at the end of the current membership period or final refill, not immediately. "If you would like to cancel your membership services or automatic, recurring shipments, for any reason, you may do so at any time in your account." Lemonaid may also suspend/terminate access unilaterally at any time without notice; if terminated for a patient's breach, no refund of prepaid recurring services.

"Last updated: September 27, 2023" (refund-policy.md). All fees and payments (memberships, auto-prescription refills, visits, or other product offerings) are stated as "non-refundable, non-cancellable, non-exchangeable, and non-creditable." No refunds for partially used services. Patient is fully liable for all account charges, including unauthorized charges (billing disputes routed to Patient Support via secure message or phone, not a self-serve refund flow).

No free-trial-period concept (e.g., "N days free") found on any public page fetched. Lemonaid instead publishes discounted first-cycle pricing per individual offer (e.g., mental health: $25 first month vs. $95/month recurring; hair-loss finasteride: $30 first 3-month supply vs. $60 recurring) — see each offer's first_vs_recurring field. No company-wide trial mechanism exists distinct from these per-product intro prices.

Terms of Use Section 4 (Updated July 10, 2026): Paid Membership can be cancelled "at any time by logging into your... Mochi account and selecting 'cancel subscription'" OR "by contacting us at (619) 648-1247 and requesting to cancel... over the phone." Explicitly: "Contacting us at hello@joinmochi.com is NOT an acceptable way to request to cancel... Requests to cancel subscriptions received via email... or through any other method not listed above will NOT be considered valid requests." Cancellation "will take effect at the end of the then current subscription term" (access continues until then); the account then "automatically revert[s] to a free membership subscription (a 'Free Membership')" rather than closing outright — Free Membership excludes 1:1 video visits and may exclude some treatment categories/Medications. Cancelling the Paid Membership also auto-cancels any active Medication Subscription that requires an active Paid Membership. Medication Subscriptions can separately be modified/cancelled "at any time prior to a refill request being sent to a pharmacy for fulfillment, by selecting 'Modify' from the medication page." If the Monthly Subscription Fee amount is going to change, Mochi must give 30 days' notice before charging the new amount, during which the user can cancel. Separate no-show policy (Patient Agreement Section 5): a missed appointment without 24 hours' notice may incur a $25.00 fee.

Terms of Use Section 4: "We do not offer refunds for partially used subscription periods, although we may provide refunds on a case-by-case basis in our sole and absolute discretion," capped at "the total fee amount paid... in the month preceding the refund request," and unavailable for any month in which the user received Mochi Provider services. Medication Fees "are not eligible for a refund, whether in part or in whole, once the order has been sent to a Pharmacy for dispensing." Separate Refunds page (refunds.md): "we cannot accept returns or issue refunds for prescription medications, as they cannot be reused or resold. Therefore, all prescription sales are final" (patient can still contact support if they believe a fill error occurred).

No time-limited free-trial concept found. Two related but distinct mechanisms instead: (1) a permanent, non-expiring "Free Membership" tier that a Paid Membership automatically reverts to on cancellation (limited features — no 1:1 video visits, may exclude some Medications/categories); (2) a new-subscriber-only first-month promotional membership price ($39 vs. standard $79/month) — see promotional/termsandconditions-first- month-promo.md and offer promo fields above. Neither is a conventional "free for N days, then billed" trial.

ToS Section 6.6: "To cancel your subscription to the Service, log into your account and follow the instructions on your account page. If you subscribed via a third party (e.g., the Apple App Store or Google Play), please visit our support center." Section 6.4: "Your subscription will automatically renew at the end of the disclosed billing period, unless canceled... You must cancel your subscription prior to 11:59 p.m. Eastern time on the day before your next recurring billing date in order to avoid being charged for the next billing period." Cancellation "will take effect at the end of the then-current billing period or free trial period... you will continue to have access to the Service through the end of your then-current billing period, unless you are subscribed through a free trial or promotion, in which case cancellation may be effective immediately." "We do not provide a refund or credit for partially used billing periods, although we may provide such refunds or credits on a case-by-case basis in our sole and absolute discretion." The Noom Refund Policy support page (faq-refund-policy.md) additionally confirms self-serve cancellation in practice: "Cancel directly in your Subscription Portal (account.noom.com/login) or in the Noom app." Same page, Q&A: "Does uninstalling the app cancel my subscription? A: No. Uninstalling the app doesn't cancel your subscription. You'll continue to be charged unless you cancel in your Subscription Portal" — a consumer-complaint-relevant fact (deleting the app is a common but ineffective attempt to stop billing). Members can also switch from Noom Med to Noom Weight in lieu of canceling outright. A separate "Noom Shop Cancellation Policy" (shop-cancellation-policy.md) governs physical merchandise/subscription-box orders from the Noom Shop, distinct from the app/Med subscriptions covered above: "you can cancel or change your subscription at any time" via links in order-confirmation emails. Collection brief context: Noom's auto-renewal/cancellation practices have drawn consumer complaints and regulatory scrutiny in the past (outside scope of this fact-only record); the ToS and support-page language above is what Noom currently publishes as its process.

Renews automatically until cancelled. ToS Section 6.4, capitalized in the original: "YOUR SUBSCRIPTION WILL BE AUTOMATICALLY RENEWED AT THE END OF THE DISCLOSED BILLING PERIOD AT OUR THEN-CURRENT FEE FOR SUCH SERVICE UNLESS YOU DECIDE TO CANCEL YOUR SUBSCRIPTION OR THE AGREEMENT IS TERMINATED" (repeated in the Terms intro, above Section 6, as a required prominent disclosure). Section 12.2: "PLEASE NOTE THAT THE SERVICE WILL CONTINUE AT THE END OF EACH SUBSCRIPTION PERIOD UNLESS YOU CANCEL YOUR SUBSCRIPTION." Noom Weight's support FAQ separately confirms: "All Noom Weight plans are billed automatically and renew unless canceled." Noom reserves the right to change prices on renewal (Section 6.5) without requiring further action/consent from the member beyond the standing right to cancel.

ToS Section 6.13: "All fees and charges assessed by us are non-refundable except as expressly promoted by Noom." California residents get a statutory 3-business-day rescission right (Section 6.7): mail a signed cancellation notice to Noom, Inc., Attn: Legal Department, One Palmer Square, Suite 441, Princeton, NJ 08542, before midnight of the third business day after signup, for a full refund. Separately, Noom's own "Noom Refund Policy" support page (faq-refund-policy.md) publishes the "expressly promoted" exceptions referenced in the ToS, broken out by product: "Noom Weight: Refunds may be available if requested within 14 days of your first charge. Renewal charges aren't refundable." "Noom Med: Refunds may be available unless a prescription has already been written. Once a prescription is written, refunds are no longer possible." "Telehealth for Branded Meds: Refunds may be available if requested within 7 days of your first charge, unless a clinical visit has occurred. Once a clinical visit has taken place, refunds are no longer possible." "Renewal charges aren't refundable, so be sure to cancel before your renewal date." Refund requests are handled via live chat ("Click the orange chat bubble... Share the date of your charge and the reason for your request. Our team will confirm your eligibility"), not a self-serve refund button.

Company-wide (ToS Section 6.3(a)): "If your subscription begins with a free trial, your first payment will be charged to your chosen payment method immediately following the end of the free trial, unless canceled... you will not receive a separate notice that your free trial is about to end or has ended, or that your paid subscription has begun, unless required by law." For Noom Weight specifically, a 7-day free trial is advertised: "7-day trial available, cancel anytime" (blog-how-much-does-noom-cost.md). No company-wide free-trial length is published for Noom Med tiers; those instead use discounted/promotional "to get started" intro pricing rather than a no-charge trial period (see each offer's first_vs_recurring). Promotions generally: "We will begin billing the same payment method we otherwise have on-file for your subscription at the then-current, non-promotional price after your promotion ends, unless you cancel prior to the end of your promotion" (ToS Section 6.3(b)).

Two mechanisms are published: (1) in the PlushCare mobile app, "My Profile" page > "Delete My Account" (ToS Section 12, Termination); (2) contacting a PlushCare Care Team member, or emailing info@plushcare.com (ToS Section 14, Membership Fees: "YOU CAN CANCEL YOUR MEMBERSHIP AT ANY TIME BY CONTACTING INFO@PLUSHCARE.COM"). Recorded as self_serve_online since the in-app deletion path exists, but email/Care-Team contact is also explicitly offered as an equally valid route. If you cancel a paid membership, it remains active until the end of the current billing term and does not renew (no immediate loss of access).

Membership fees: non-refundable in the ordinary case ("All fees are in U.S. Dollars and are non-refundable," ToS Section 4). Exception: if PlushCare terminates your access for a reason other than your own breach, you receive a pro-rata refund of fees paid; if PlushCare terminates for your breach, no refund. Visit/appointment fees: full refund if PlushCare is unable to treat/assess you, connectivity issues prevent the consult, the doctor doesn't show within 10 minutes, a state licensing issue blocks the consult, or insurance verification was pending/unsuccessful at booking — refund excludes membership fees, medication, and lab costs. ToS Section on Billing ("Automatic Renewal Program"): "If you wish to cancel an Automatic Renewal Program, you may do so at any time by calling us at: (866) 294-3772. For all California customers, all New York customers, and select Vermont customers having an initial subscription of one year or longer, you may also cancel an Automatic Renewal Program by: (i) signing in to your member portal; or (ii) e-mailing us at: support@rexmd.com." Phone is the default/general mechanism company-wide; self-serve online (member portal) and email are available only to the carved-out CA/NY/select-VT ≥1-year-subscription population. Separately, "all automatic renewal program subscription orders must be cancelled within (12) hours of the renewal subscription date to avoid the order being shipped and you incurring charges for same" — a notably short cancel-before-renewal window. Rex MD's own FAQ (faq.php) states more broadly, without the ToS's state carve-outs: "We offer the flexibility to pause or cancel your subscription at any time. Simply reach out to our customer Care Team at [phone], [email], or through your patient portal to make any changes to your account" — this FAQ language implies portal-based self-cancellation may be available to all patients, which is broader than (and in tension with) the ToS's explicit CA/NY/VT-only carve-out for the portal option. Recorded cancellation_mechanism as phone_required per the ToS (the binding legal document and the more specific/restrictive statement); flagging the FAQ's broader claim as needs_recheck.

Renews automatically until cancelled. "Automatic Renewal Program" is the named subscription model in the ToS: Payment Method is charged Recurring Fees on a recurring basis for as long as the subscription remains active, with no additional authorization sought for each renewal ("Every time that you accept delivery of the subject Products, you re-affirm that REX MD® is authorized to charge your Payment Method").

"UNLESS OTHERWISE INDICATED, ALL SALES ARE FINAL AND NON-REFUNDABLE" (ToS, General Billing Terms). Return Policy (Section 6): non-prescription products may be returned via customer care; "Prescription Medications are not eligible for return or refund." Exception, per faq.php: "If you are signing up for a treatment plan that may include a prescription, you will be charged once completing your initial onboarding questionnaire. If for any reason your provider determines you are ineligible for treatment or medication, you will receive a full refund." Separately, faq.php states refunds otherwise are handled "on a case-by-case basis" via support@rexmd.com. ToS Section 5 (Products) separately guarantees a refund if Rex MD itself cancels an order due to a pricing error or unavailable inventory.

No free-trial-period concept ("N days free") found on any public page fetched. Rex MD instead advertises discounted/promotional per-product pricing (e.g. hair-loss finasteride "Save 50% Off," TRT "$99, discounted from $199," site-wide ED "SUMMER... New customers are only charged after completing the intake questionnaire, and receive a full refund if a provider determines they're ineligible for treatment (faq.php) — this is a charge-only-if-approved model, not a trial.

Two documented paths, differing by product. Ro Body (weight loss) membership — ToS: "To cancel your Ro Body Program membership, you may log into your Secure User account (click into the Ro Body Program and select 'Cancel Subscription' from the Program Details page) or by emailing us at care@ro.co. Message your provider if you want to discuss switching medications while remaining in the Ro Body Program. To avoid further charges to the payment method on file, you must cancel at least 48 hours in advance of your next renewal date." Effect of cancellation: "If you cancel at least 48 hours before your next renewal date, your Ro Body Membership will not automatically renew on your renewal date, and you will not be charged any additional Ro Body Membership Fee payments. You will receive Ro Body Program Membership Services through the last day of your billing cycle for which you have paid... Thereafter, you will not be eligible to receive any Ro Body Program Membership Services, including prescriptions from your provider... You may be required to complete a new Ro Body Program Diagnosis Online Visit to talk to a Ro-affiliated healthcare professional about re-joining." All other subscription products — FAQ (Cost, pricing & services): "You can easily pause or cancel your order by visiting our Contact Us page. Turn off automatic shipping: You can stop future orders from shipping automatically by turning off your auto-refill. Delay shipping: You can change your automatic shipment date by delaying your auto-refill. Cancel a processing order: If your order is currently processing, please contact us as soon as possible. Our Care Team will try to assist you as quickly as possible. Cancel a shipped order: Once your order is shipped, we cannot return it." General ToS clause: "You may cancel a Subscription Service to prevent future renewals at any time before the cancelation cutoff date as indicated to you at the time you purchase a Subscription Service. You will also be informed about how you can cancel these Subscription Services." The generic cutoff date is not stated in the ToS itself. Ro also publishes a company post ("How Ro puts patients first by making it painless to cancel", Zachariah Reitano, updated Aug 26, 2019) stating: "We send an email before every single shipment to make sure the patient still wants or needs their treatment (not after it's been shipped)... And if they want to reach out via email or via phone to cancel, that's okay too." Ro Body Care Team hours: Monday-Friday, 9:00am-6:00pm ET, excluding holidays (care@ro.co).

Renews automatically until cancelled. ToS, Ro Body: "If medication is prescribed, you will be enrolled in the automatically renewing Ro Body Program Membership... and your payment method will be charged on an automatically renewing basis unless you cancel in accordance with these Terms." And: "Your payment method on file will be automatically billed for the cost of Membership at the price and frequency displayed at the time of checkout on an automatic, recurring, basis unless and until you cancel." And, in caps: "YOU WILL BE AUTOMATICALLY CHARGED THE RO BODY MEMBERSHIP FEE EVERY MONTH EVEN IF YOU DO NOT USE ALL OF THE OFFERED RO BODY MEMBERSHIP SERVICES. Even if you do not request or receive medication through the Ro Body Program, you will still have access to the Ro Body Membership Services described above and you will continue to be charged on an automatically renewing basis unless and until you cancel. We reserve the right to cancel your membership at any time with reasonable notice." ToS, general subscriptions: "Certain products or services offered on the Services may be offered on a subscription basis ('Subscription Services'). For Subscription Services, your payment method will be automatically charged at regular intervals as described during the checkout process." Skincare: "After your first order, you will automatically be shipped a new order and billed every 2 months—$58 for Custom Rx Treatment and $16 for Enriching Cream—until you cancel." Price-change clause: "Prices are subject to change at any point in our sole discretion."

Returns & Refunds page (full text): "Unfortunately we cannot issue refunds or accept returns of prescription products for reuse or resale, and all sales are final. However, if you feel we have made an error in the filling of your prescription, please contact us." ToS adds automatic-refund cases: (a) "You will automatically be refunded if we have already charged your payment method and you are subsequently deemed unsuitable for treatment or we reject your treatment request for any other reason. These refunds... may take 5 business days or longer to process." (b) an expired treatment request is automatically refunded. (c) if you accept a provider-suggested alternative treatment plan, "the initial charge will be refunded automatically, and you will incur a new charge for the alternative treatment plan." Ro Body membership specifically: "If medication is never prescribed, you will be refunded your purchase price and you will not be enrolled in the automatically renewing Ro Body Program Membership." The pricing page states "Risk-free start: Your $39 will be refunded if you're not eligible for GLP-1s." But once billed: "Once paid, your Ro Body Membership Fee is non-refundable." Lilly GLP-1 products: "Any refunds, replacements, or disputes related to Products are subject to the policies and review of the dispensing pharmacy and, where applicable, Lilly." — i.e. not Ro's policy.

No free trial published. Ro uses paid introductory pricing instead: - Ro Body: "$39 first month" then $74-$149/mo. ToS: "Ro may offer a one-time introductory price ('Introductory Price') when you first enroll into the Ro Body Program. The Introductory Price will be governed by the terms and conditions disclosed to you at checkout." Refunded if not eligible for GLP-1s. - Custom Rx skincare: 25% off first order ($43 vs $58 per 2 months); the product page itself calls this "the trial offer." - ED: footnote on /erectile-dysfunction/ reads "*If prescribed. Offer valid only for new sexual health patients on a recurring plan. Minimum $35 order value." The headline offer that this footnote qualifies did not render in the scraped page text (it sits in a banner/image). See needs_recheck.

Fallback for either: email support@sesamecare.com or call +1 (877) 947-6411 (faq.md, terms-of-service.md). Subscriptions (Membership Services, and by extension Success by Sesame / Skincare Rx program subscriptions) can be canceled "at any time up to forty-eight (48) hours before the applicable renewal processing date." After cancellation, access continues through the end of the then-current billing period; the subscription does not renew again, and no prorated refund is given for the remainder of that period. Separately, the Success by Sesame weight-loss program has its own stated rule: full refund only if canceled at least 3 hours before the FIRST/initial visit; no refund for that first month once the initial visit has occurred.

Renews automatically until cancelled. Subscriptions (Membership Services, and the same recurring-billing language governs the Success by Sesame / Skincare Rx program subscriptions) auto-charge the payment method on file each period "until you cancel" (ToS Section 4, Membership Program). If Sesame raises a subscription's recurring fee, it commits to giving "advance notice" and letting the member choose to continue or cancel before the new price takes effect.

Per-visit refunds (faq.md / membership-perks.md, consistent across both): full refund if a virtual appointment is canceled at least 3 hours before its scheduled time, an in-person appointment at least 24 hours before, or (for labs/imaging/other appointments without a fixed date/time) within 10 days of purchase once the provider confirms the appointment did not take place. No refunds for: patient no-shows; cancellation requests inside those windows; requests to prescribe a specific medication or complete specific paperwork that is declined; surgical procedures with confirmed scheduling; labs not completed within 7 days of purchase; any refund request made more than 10 days after the scheduled visit. Subscription refunds: monthly/annual Membership Services subscriptions are not eligible for a prorated refund upon cancellation (access simply continues to the end of the paid period). "From time to time Sesame may offer free trials related to Membership Services for a specific period of time." Only the membership subscription fee itself is waived/discounted during any trial period — "all other associated fees (i.e. provider visits, labs, etc.) will be charged during the Trial Period." No standard/default trial length is published; terms are set per-offer at the time a specific trial is run. If a trial converts to paid, the member can cancel by email or phone before conversion to avoid being charged.

Terms of Use, Section 20 (Subscription Policy): "All of our prescriptions are subscriptions to help you stay on track with your medication. You will be charged the full amount for your prescription every 30 or 90 days, depending on your refill schedule, until you pause or cancel your subscription. You can pause or cancel your subscription at any time by emailing care@struthealth.com." FAQ (Subscriptions & Pricing): "Does Strut allow you to cancel your subscription? Yes, we offer convenient flexibility to cancel, pause, or delay your monthly subscription order." "Can I change the frequency of my orders? ... Log into My Account and adjust your order under the Orders and Subscriptions tab." "Can I delay my next shipment? ... Log into My Account and adjust your order under the Orders and Subscriptions tab." FAQ (Orders & Shipping/return policy): "Subscriptions can be cancelled with 48 hours notice before processing." The FAQ describes self-serve account-level controls for pause/delay/ frequency changes, while the Terms of Use names emailing care@struthealth.com as the subscription pause/cancel method; both mechanisms are recorded here since the pages do not reconcile which is authoritative for cancellation specifically (account controls were not tested — Phase 0 rule 1 forbids entering the authenticated account area).

Refund Policy (last modified September 21, 2021): "For all medications that leave the pharmacy, we are unable to accept returns or offer refunds due to government regulations. We cannot do refunds for any cancellation requests received after the pharmacy has processed your order." Exchanges: "We only replace items if they are defective or damaged and must contact us within 2 weeks of delivery. ... All returns of medication must be sent back to the dispensing pharmacy. Strut cannot receive any returns." FAQ: "Prescriptions cannot be returned once they leave the pharmacy. Contact our customer service team and we will do our best to provide you with a solution."

No published free trial for medication. Company-wide "free" claims apply to ancillary services, not the drug cost: free online MD visit, free follow-up care/ provider messaging, and free discreet shipping on every order (Our Story, How It Works). "Some states do not require phone consultations" (FAQ) — the initial online visit itself is described as free regardless.

Dedicated cancellation page (weightwatchers.com/us/cancellation): "if you'd like to cancel or switch your membership, you can do it here" — links to a self-serve flow at cmx.weightwatchers.com/cancellation; also offers 24/7 chat for assistance. Same page states when cancellation takes effect: for multi-month Commitment Plans, at the end of the Commitment Plan period (after which, cancellation takes effect at the end of the membership month in which you cancel); for pre-pay Savings Plans, at the end of the initial plan period (same after-period rule applies); for standard one-month plans, at the end of the membership month in which you cancel. The separate WW Clinic Membership Agreement (ToS Section 3.B) additionally lists alternate cancellation channels: "selecting that option in the protected areas of the Site or Mobile App, emailing hello@joinsequence.com, contacting your Care Team, contacting Customer Service, or by such other means as we may provide from time to time." Cancellation requests are attempted to be processed "within 72 hours." Company reserves the right to collect fees, surcharges, or costs incurred before cancellation takes effect.

Renews automatically until cancelled. Confirmed both in every pricing promo's fine print ("Plan auto-renews for another 12-month period at the same monthly rate at the end of each plan term unless canceled") and in the ToS itself (Section 2.C / Section 3.A): recurring/commitment plans auto-charge at the start of each renewal period unless canceled beforehand; the company may modify subscription fees with at least 30 days' prior written notice.

The subscription fee is non-refundable/non-cancellable once a billing period (standard monthly plans) or commitment period (long-term commitment plans) has started — a canceling member must pay the applicable fee through the end of that period. Two carve-out exceptions: (1) if membership is canceled for a MEDICAL reason before the end of a paid period, the unused portion is refunded (excluding any fixed upfront fee, e.g. initial consultation fee); (2) if THE COMPANY terminates a membership (not the member canceling, and not for a member's breach), the unused portion is refunded pro rata (same upfront-fee exclusion). Company may deduct an administrative-cost fee from any such refund. Apple iTunes in-app purchases are separately non-refundable per Apple's own iTunes ToS. Medication shortages do not trigger refunds ("Members will not be refunded any fees, even if prescribed medications are unavailable, except as otherwise permitted in this Agreement").

No free-trial-period concept (e.g., "N days free, cancel before you're charged") was found for the Clinic/Med+ product on any page fetched. Instead, WW uses a discounted INTRODUCTORY MONTHLY RATE tied to a required 12-month or 6-month plan COMMITMENT purchase (e.g. $25/mo for the first 1-2 months, converting to $74/mo or $84/mo for the remainder of the committed term) — this is a discount-with-commitment structure, not a cancel-during-trial-for-free structure, since the ToS subscription-fee refund rules above still apply once the plan is purchased.
